CrestPoint Health will discontinue offering Medicare Advantage plans starting June 1, 2016.  This means your coverage through CrestPoint Health Medicare Advantage Classic and Value plans will end May 31, 2016.  You will need to make a decision about your Medicare coverage and select new coverage.  If you don’t take action before June 1, 2016, you will be enrolled in Original Medicare and a standalone Medicare Prescription Drug plan.

You will receive an Important Notice in the mail in the coming weeks providing information about your options and the actions you must take. If you do not receive your Important Notice within 2 weeks of this posting, please call us at 866-828-3307, Monday through Friday from 8:00 a.m. to 8:00 p.m. Eastern time. TTY Users call 800-848-0298. Tell the customer service representative you did not receive your Important Notice.
